Athletic Health & Safety Requirements
At Trinity High School, the health and safety of our student-athletes is a top priority. To ensure every athlete is properly prepared for participation, all required emergency and medical forms must be completed prior to the start of each sports season.
By completing these forms, families provide essential medical and emergency contact information directly to our athletic trainer, Sara Horne (slhorne@thsstj.org). Athletes must also submit a current physical examination, dated within the past year, before they are cleared to participate.
Required Forms (Due Before Participation):
All student-athletes must complete and submit the following prior to the first day of practice:
- Emergency Information Form
- Permission to Participate
- Code of Conduct
- Concussion Agreement
- Current Physical Form (completed by your physician)
Please note: There is no online link for physical forms. Athletes must submit the physical form provided by their healthcare provider.
Sports Physicals Made Easy:
If your athlete needs a current sports physical, Trinity’s partner, ExpressMED, offers FREE sports physical evaluations at their downtown Manchester location.
Concussion Baseline Testing:
To support athlete safety and concussion management, two baseline concussion tests (ImPACT and C3 Logix) are required for all contact/collision sports:
- Prior to an athlete’s freshman year
- Prior to an athlete’s junior year
Baseline testing is also required for:
- Students transferring to Trinity High School
- Athletes participating in a contact/collision sport for the first time
Refund Policy:
Refund requests due to injury may be submitted within two weeks from the start of the season’s first official practice. After this period, refunds will not be issued.
